Welcome to Avenues Host Home Program Toolkit!
The materials we offer here have been developed, tested, adapted, revised, used and shared for 20 years. YouthLink started the GLBT Host Home Program in 1997 thanks to what we learned from other communities. The program was renamed ConneQT in 2019. And we are happy to continue sharing what we have learned with you.

Avenues’ ConneQT Program is a community-based approach to providing housing to youth ages 16-24, while also building community and resources. Below you will find materials and information as well as sample forms for the ConneQT Program.. We hope these are helpful! Feel free to use and change them to meet the needs of your community.
